5 P: 1 countywide contract to identify a lead partner (or partnership) to work with ASC, as well as with care service providers, to improve links to the voluntary sector and preventative measures in care. Support to be offered to at least 1 200 people on an annual basis. Volunteers to be utilised in service delivery as appropriate.

5 P: 1 contract to include the following elements:

5 P: — community assets — info and advice in a variety of formats on local community assets/resources. Help with the development of the pathway from social care to the voluntary sector (e.g. supporting social care teams and care providers to understand how to link people to local community resources) and to offer advice to smaller voluntary organisations on applying for grant funding (e.g. identifying what is needed at a local level and applying for social inclusion funding). The provider will be expected to make best use of existing community resources, such as developing links to Community Makers, Community Care-Navigators, Care-Coordinators and Social Prescribing Link Workers,

5 P: — inclusion plans — help for people who approach ASC for support (including the access service) but are unlikely to be eligible for social care to develop an “inclusion plan”. This will ensure the person makes links to local resources and support networks to improve independence and wellbeing. People who are eligible for social care will be supported by social care teams to make links in the community where appropriate, but social care staff may contact the provider for help and guidance, or to make a referral if felt to be more appropriate to meet the needs of the person. Referrals will also be accepted from other sources, including but not limited to other voluntary sector organisations and health professionals as appropriate,

5 P: — wellbeing support — the provider will offer ongoing support from volunteers to people in the community experiencing a risk to their wellbeing and independence, either on a one-to-one or group basis as appropriate. This will include emotional support, guidance on improving wellbeing, low level practical support with necessities (e.g. check the heating is on, water is working, there is food in the cupboard), as well as help to access local support networks and make links to the local community to reduce loneliness and social isolation. This will include support from volunteers for people during and after leaving hospital or another placement (e.g. temporary care home placement) on return home. Funding to be used to recruit, train and supervise volunteers, as well as to cover volunteer expenses (e.g. mileage),

5 P: — volunteering opportunities — identifying opportunities for people experiencing a risk to their wellbeing and independence to be volunteers and supporting people to access and maintain volunteering opportunities. Working with local businesses to consider volunteering opportunities for people with additional needs. Helping people to prepare for their volunteering opportunity and ensuring that they are managing and whether any other support is required for the person to ensure the volunteering opportunity continues.

Referrals from ASC, Housing services and other Council services to be prioritised but people do not need to be eligible for social care to access the support.

5 P: The service specifications will set out the following requirements:

5 P: — autism (including high functioning autism and Asperger’s),

5 P: — physical health (including physical disabilities, mobility issues, HIV and other long term conditions),

5 P: 2) Independent living support: this will include linking people to existing community resources as well as offering low-level practical support with daily living e.g. form filling, utilising online self-help tools and providing non-statutory advocacy where required.

5 P: 3) Self-management support: to help people to learn techniques to self-manage their health and wellbeing, including preventing a fall and staying physically active and mentally stimulated.

5 P: 3) Flexi budget: additional 5 % to be included in the contract. The primary purpose of this budget is to offer a small amount of funding to be used to help with any aids for daily living or self-management, but will be used flexibly to meet personalised needs.

5 P: 5 locality-based supported accommodation and community outreach contracts. Support for at least 500 people on an annual basis on a one-to-one or group basis. To include 220 units of short term supported accommodation (typically no more than 2 years) and short term community outreach (typically no more than six months) for at least 280 people. Referrals from health and social care agencies, Housing and other public sector services to be prioritised but people do not need to be eligible for social care to access the support.

5 P: 2) Independent living support: this will include linking people to existing community resources as well as offering low-level practical support with daily living e.g. form filling, utilising online self-help tools and providing non-statutory advocacy where required.

5 P: 3) Self-management support: to help people to learn techniques to self-manage their health and wellbeing, including self-managing mental distress and alcohol/drug use.

5 P: 4) Flexi budget: additional 5 % to be included in each contract to offer a small amount of funding to be used to help with any aids for daily living or self-management. The primary aim of this budget for people in supported accommodation is to support move on. However, it is to be used flexibly to meet personalised needs.

5 P: 5) Short term supported accommodation to be included for people who are unable to access accommodation in the community due to high levels of risk/need. The contracts relate to the revenue funding for the support service only and do not cover rental, housing management or capital build costs for the accommodation:

5 P: — high/medium tolerance: this component of the service will provide high to medium levels of tolerance to drugs and alcohol and high to medium levels of support,

5 P: — limited tolerance: this component of the service will provide limited levels of tolerance to drugs and alcohol and high to medium levels of support. It is for people who have changed their behaviour in relation to drug/alcohol use but still need some flexibility to maintain this change, as well as people that have not had drug/alcohol problems, including but not limited to people with mental health needs only.
