Summary

What is the problem/issue being addressed?
RITMOCORE will transform the care path of patients with bradycardias, implanted, or to be implanted with a pacemaker, most of them aged over 70 and with multiple comorbidities. The motivation for transforming it is the steady growth of the demand -mostly due to the ageing population- and budgets stability, which do not allow to respond to such demand increase. The consequences are two-fold, on one hand there is a need of more devices and, on the other hand, the increase in the workload of the clinicians becomes unsustainable.
The RITMOCORE model addresses both problems: how to purchase more devices, and how to make the best use of clinicians’ time, while increasing quality of care and contributing to preserve long term sustainability of the healthcare system. All these is enabled by the use of advance ICT resources.
Why is it important for society?
Increasing demand with same resources entails budgetary tensions for the healthcare insititutions, but it is also relevant, by extension, to the whole healthcare system because this problem challenges the sustainability of the system at mid-term. Furthermore, the quality of care, patients\' and relatives\' satisfaction is also at stake, since although technology enables a close follow-up, solutions not always guarantee the best results for the end users when they are upscaled.
What are the overall objectives?
RITMOCORE designs and implements a needs-driven joint procurement procedure to move from a conventional purchasing of devices to an patient-centred service provision including: the set up of a remote monitoring system capable to meet the procurers\' coverage area needs; the selection and provision of the appropriate treatment for each patient; the definition and implantation of information flows to maximise the value of the data retrieved from devices to facilitate a patient-centred model implantation; the setup of a patient activation programme for pacemakers recipients and relatives; the implementation of a change management programme for the involved healthcare institutions and the implantation of a risk sharing model that aligns the interests of end-users, service providers, healthcare institutions and payers.

Work performed

During the preparation stage many efforts have been devoted to the consolidation of the buyers group, by adding new procuring entities to the consortium, and aligning the vision of the different stakeholders (clinicians, managers, procurement offices) from different institutions.
The alignment above has allowed the consortium to define a common challenge that sets the foundations for a new value-based model of care for pacemaker patients as well as an innovative pacemaker-related services provision which meets the needs raised by patients, relatives and health professionals.
RITMOCORE\'s value-based model consists in the provision of comprehensive services intended to satisfy the unmet needs through the implementation of different pillars: change management, remote monitoring, personalised therapy, coordination of care, patient activation and risk sharing provisions.
The definition of the model is part of the early stage of the procurement process and it has enabled the consortium to elaborate the tender specifications including the definition of the awarding criteria and to conduct in parallel the major steps in a procurement procedure, such has the publication of the Prior Information Notice, dated on July 2018, and the realisation of an open market consultation (OMC). OMC is a key part of the process as it serves to receive input from the market to check the viability and feasibility of the model as well as to enrich the procurement process in a transparent way.
Besides the OMC activities, mainly targeting potential bidders, the consortium has also dedicated efforts in disseminating the project model among other stakeholders such as other healthcare providers, regional healthcare authorities or industry associations.
The elaboration of the tender documents is the main result achieved during the preparation stage, but this main output required a number of intermediate results including the definition of a business case at different levels from the public perspective, the definition of an evaluation framework to assess the innovation of the bids that will be eventually received, the performance of the service and its societal impact; the creation of a quality labelling procedure, intending to maximise the value brought by many solutions delivery to the market, and a model of internal organisation for the procuring entities.

Final results

RITMOCORE has completed the preparation stage of its cross-border joint procurement process, which is a major milestone in the innovation procurement scenario. Meanwhile, the consortium has defined a new service model that potentially maximises the value brought by the solutions providers by leveraging new business models more favourable to ensure the sustainablity of healthcare systems, which will be evenutally benefit from upscaling the new service provision.
The upcoming execution stage will reveal the results and impacts of the service implantation, although the experience cumulated so far in preparing such an unique process (in the healthcare sector) has a huge potential impact for those insitutions intending to use innovative procurement across Europe.
